Qualifications

• MSN required; DNP or related doctorate preferred.

• Experience serving underserved, unhoused, or marginalized populations.

Responsibilities

• Deliver primary care across the lifespan in mobile, church-based, and elementary school settings throughout South Los Angeles and South San Diego County.
• Manage day-to-day operations across a weekly rotation of community clinic sites.
• Supervise Nurse Practitioners and Community Health Administration staff.
• Oversee scheduling, workflows, EHR operations, billing, credentialing, and quality improvement.
• Support federal grant compliance, reporting, outcome tracking, and budget monitoring.
• Build partnerships with schools, faith-based organizations, and community agencies.

Must reside in California (San Diego preferred) | Regular travel to Los Angeles

A university-affiliated, nurse-managed community clinic is seeking a mission-driven Family Nurse Practitioner to lead and deliver care across its community health programs in San Diego and Los Angeles. This hybrid clinical-leadership role combines direct patient care with clinic operations, team supervision, and grant program management, serving people experiencing homelessness, veterans, school-age children, and other underserved communities.

This position is grant-funded through June 30, 2027.

What You'll Do

• Deliver primary care across the lifespan in mobile, church-based, and elementary school settings throughout South Los Angeles and South San Diego County
• Manage day-to-day operations across a weekly rotation of community clinic sites
• Supervise Nurse Practitioners and Community Health Administration staff
• Oversee scheduling, workflows, EHR operations, billing, credentialing, and quality improvement
• Support federal grant compliance, reporting, outcome tracking, and budget monitoring
• Build partnerships with schools, faith-based organizations, and community agencies

Requirements

• MSN required; DNP or related doctorate preferred
• Active FNP certification (ANCC or AANP), unencumbered CA RN and APRN license
• Active CA furnishing number, NPI, and DEA registration
• 5+ years of healthcare leadership or management experience
• Pediatric experience required
• Experience serving underserved, unhoused, or marginalized populations
• Willingness to travel up to 75%, with mileage and travel reimbursed

• Street medicine, mobile clinic, or school-based health experience
• Nurse faculty or preceptor background
