Salary: $95,000 – $115,000 annually

Position Overview

The Nurse Practitioner will provide comprehensive primary care services, including health assessments, diagnosis and treatment of acute and chronic conditions, preventive care, patient education, and care coordination. This role works closely with a multidisciplinary healthcare team to deliver evidence-based, compassionate care to diverse patient populations.

Responsibilities

• Conduct comprehensive patient assessments, including medical histories and physical examinations
• Diagnose, treat, and manage acute and chronic health conditions
• Order, perform, and interpret diagnostic tests and screenings
• Prescribe medications and treatments in accordance with state regulations and organizational policies
• Provide preventive healthcare services, screenings, and wellness counseling
• Educate patients on disease prevention, treatment plans, medications, and healthy lifestyle practices
• Develop individualized care plans based on patient needs and evidence-based guidelines
• Collaborate with physicians, nurses, behavioral health providers, and other healthcare professionals to coordinate patient care
• Maintain accurate and timely documentation within the electronic medical record (EMR) system
• Promote quality patient outcomes through evidence-based practice and patient-centered care

Qualifications

Required

• Current Nurse Practitioner license in the state of Pennsylvania
• Graduate degree from an accredited Nurse Practitioner program
• Strong clinical assessment, diagnostic, and treatment planning skills
• Knowledge of evidence-based clinical practice standards
• Excellent verbal and written communication skills
• Ability to maintain patient confidentiality and professional boundaries
• Experience utilizing electronic medical records and clinical documentation systems
• Strong organizational and time management skills

Preferred
