Job summary

Project HOME in Philadelphia, PA seeks a Family Nurse Practitioner to join its integrated healthcare team and deliver primary care to children, adults and OB/GYN patients. This salaried role offers a Monday–Friday schedule and a base salary of $113,775–$117,757 per year, plus full benefits.

Qualifications

• Master's Degree and current NP license (Family Nurse Practitioner required) with three years of practice serving gynecology, obstetrics and pediatric patients.
• CPR and DEA certification.
• Experience serving underserved populations.
• Experience with trauma, housing insecurity, or related social determinants of health.
• Strong communication, clinical and organizational skills.
• Experience with EHR systems such as eClinicalWorks.

Responsibilities

• See 9 patients per session, 8 sessions per week, with holistic and trauma-informed care.
• Collaborate with medical assistants, behavioral health, care managers and peer specialists.
• Address clinical and social determinants of health for marginalized populations.
• Uphold ethical, equitable, and evidence-based care.
• Participate in quality improvement, staff development and clinical leadership.
• Mentor NP, PA and MD students and engage in CME.
