Job summary

Terros Health in Phoenix, AZ is recruiting a Family Nurse Practitioner with Pediatrics experience for our 27th Avenue Health Center. This full-time role involves in-office patient care, conducting exams, evaluating health, and coordinating with a diverse care team.

The ideal candidate holds an AZ NP license, a Master’s or Doctorate in nursing (FNP), and BLS; DEA license and ANCC board certification are required. PX location near 27th Ave/Indian School Rd; Spanish or other language skills a plus.

Qualifications

  • Active AZ NP license with Family Nurse Practitioner specialization.
  • Master's or Doctorate in nursing; FNP specialization required.
  • Valid BLS certification and current DEA license with ANCC board certification.
  • Arizona driver's license, 21+ with 4+ years driving experience; Terros Health policy.
  • Arizona Fingerprint Clearance Card required or applied within 7 working days.
  • Pass drug screen, TB test, and background check as part of onboarding.
  • Proficient with NextGen and MS Office; computer skills necessary.

Responsibilities

  • Perform physicals/check-ups, evaluations, and diagnostics within primary care.
  • Prescribe and monitor medications; coordinate with other providers.
  • Lead and supervise clinical staff as needed; ensure documentation quality.
  • Educate patients on preventive care, wellness, and treatment plans.
  • Maintain accurate patient records and comply with licensing requirements.

Salary Range (based on 40 hours/week): MSN: $115K-$120K+ DOE; DNP: $129K

Additional Language Differential Pay Available: Spanish,Vietnamese, Korean, Arabic, Farsi, Swahili or ASL: $3/hour ($6240 annually)

The Primary Care Providerconducts physicals/check-ups, medical evaluations, diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, coordinates with other providers, makes specialty care referrals, educates patients about preventative care, establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate treatment goals, within the context of an integrated setting.

Duties include:
  • Performs independently or as a part of a treatment team.
  • Conducts physicals/check-ups, evaluations, diagnostics, treats illnesses, orders lab tests, general and preventative care, medication prescribing and monitoring, and individual treatment and case consultation.
  • Provides the important task of educating patients about preventative care, wellness activities and prescribed treatments.
  • Provides leadership and cohesion within the clinic environment, and may provide direct supervision to other professional and/or non- professional medical staff, and students.
  • Participates in program evaluation procedures and professional record keeping, makes referrals appropriately for specialty and behavioural health care.
  • Provides direct patient services not less than 85% of the work time.
  • Employs therapeutic interventions appropriate for identified treatment issues.
  • Seeksappropriate clinical supervision/support, when necessary, to ensure the provision of effective treatment.
  • Establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations

Qualifications
  • 1. Nurse Practitioner AZ License
  • 2. Master’s degree or Doctorate in nursing (Family Nurse Practitioner)
  • 3. Possess a valid BLS certification
  • 4. Possess a valid DEA license and Board Certification (ANCC)
  • 5. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
  • 6. Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • 7. Must pass a Drug Screen, TB Test and Background Check.
  • 8. Proficient using computer-based systems including electronic health records (NextGen preferred), Microsoft Outlook, and other Microsoft Office products.
