Job summary

Yoh Services LLC is seeking a Family Nurse Practitioner for a contract opportunity at its Primary Care clinic in Hammond, IN. The schedule is structured with multiple daily shifts and a potential extension or direct placement beyond 12/28.

• Active Nurse Practitioner license to practice in the state of Indiana.
• BLS certification required at hire date.
• State CDS registration and DEA registration current.

• Develops and documents patient care plans and progress notes.

Family Nurse Practitioner

Contract opportunity with Yoh's Primary Care clinic located in Hammond, IN.

Schedule

Tues & Wed: 9a-7p, Thurs & Fri: 6a-4p

Duration

10/12-12/28 with possible extension or direct placement

Pay

$65-78/hr DOE

What You'll Be Doing

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You Need to Bring to the Table

• All nurse practitioner licenses/certificates to practice
• BLS upon hire
• State-specific Controlled Dangerous Substances (CDS) registration
• Current DEA registration
• 3+ years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice, urgent care, emergency department, or related setting
