A supportive community organization is seeking a Family Model Provider in Chattanooga, Tennessee. This independently contracted role involves transforming your home into a safe and caring environment for individuals with developmental and/or physical disabilities. Applicants should have a high school diploma, valid driver's license, and experience as a Direct Support Professional. Full reimbursement and monthly support from the individual are provided. We are now hiring someone in the Chattanooga, TN area. We are seeking a person who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high‑quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family‑like setting for an individual.

This role will care for an individual in need of assistance with medical appointments and minor assistance with grooming.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
