A supportive community organization is seeking a Family Model Provider in Chattanooga, Tennessee. This independently contracted role involves transforming your home into a safe and caring environment for individuals with developmental and/or physical disabilities. Applicants should have a high school diploma, valid driver's license, and experience as a Direct Support Professional. Full reimbursement and monthly support from the individual are provided. We are Now hiring someone in the Chattanooga, TN area. We are seeking someone who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual.

This role will help care for an individual in need of assistance with medical appointments and minor assistance with grooming.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our FamilyModel Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
