Job summary

Adelante Healthcare in Phoenix, AZ is seeking a registered nurse to provide triage support within a patient-centered primary care setting. You will collaborate with care teams, coordinate referrals, and help educate patients on health plans and preventive services.

Ideal candidates have 2–5 years clinical experience, AZ RN license in good standing, CPR/AED certification, and strong computer skills for patient management systems. Valid AZ driver’s license required.

Qualifications

• Graduation from an accredited nursing program.
• Licensed with AZ State Board of nursing; license in good standing.
• 2-5 years of experience in a clinical setting.
• Certification to perform CPR and AED (AHA/Red Cross guidelines).
• Advanced computer efficiency in Microsoft Office and experience in Patient Management System.
• Valid Arizona driver's license.

Responsibilities

• Performs triage calls using approved clinical decision support tools for adult primary care, OB, and pediatrics.
• Participates in team meetings and quality improvement initiatives.
• Facilitates patient education and coordinates continuity of care with external healthcare organizations.
• Coordinates outreach to high-risk patients and hospital discharge planning.
• Reviews and interprets lab results and assists with patient exams and care plans.
