Job summary

Jericho Road Community Health Center is seeking a full-time Healthy Steps Program Specialist in Buffalo, NY. You will work across Barton, Broadway, and Doat locations within the Healthy Steps Department, conducting assessments, coordinating referrals, and collaborating with a multidisciplinary team to promote holistic family wellbeing.

Responsibilities include education of families, documentation in Medent EMR, ongoing professional development, and participation in community Healthy Steps

Qualifications

• Experience with children and families in education, mental health, or healthcare settings.
• Understanding of family systems theory and dynamics.
• Strong interpersonal and communication skills and ability to engage families.
• Ability to work with diverse populations with cultural sensitivity.
• Commitment to client-centered and family-centered care.

Responsibilities

• Conduct comprehensive assessments of children and families to identify developmental and behavioral concerns.
• Coordinate referrals, appointments, and follow up to reduce barriers to care.
• Collaborate with an interdisciplinary team to create integrated care plans.
• Educate and support families on child development and parenting skills.
• Maintain accurate documentation in the Medent EMR and ensure regulatory compliance.
• Engage in professional development and participate in community Healthy Steps activities.
• Support site-level inventory management for Reach Out and Read.
