Qualifications

• Bachelor's degree required.
• Minimum 2 years' experience with parenting, child development, social work or human service work.
• English proficiency (speaking and writing) required.
• BLS Certification through the American Heart Association required at time of hire and maintained.
• Bilingual language requirement: Spanish or Mandarin or Haitian Creole fluency; written proficiency a plus.

Responsibilities

• Conducts office visits and developmental screenings in collaboration with the pediatric care team.
• Works closely with the pediatric team as a resource for developmental and parenting concerns.
• Facilitates parents/caregivers education and mental wellness groups.
• Participates in reflective supervision meetings.
• Provides anticipatory guidance to parents and other primary caregivers on early learning, sleep, feeding, etc.
• Documents all patient activity and care coordination in patient records and data systems.
• Provides referrals and tracks follow-up to connect families with services when delays or concerns are identified.
• Works with pediatric providers around care coordination, goal setting, counseling and education.
