Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator in Pickaway, OH to link services and coordinate care for children, youth, and families with significant behavioral health needs. The role emphasizes collaboration with Child and Family Teams and community agencies to support voice and choice in services.

The position offers a $3,000 hiring bonus, a competitive hourly pay range, and a strong benefits package.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Knowledge of Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
• Ability to operate in an Internet-based, automated office environment.

Responsibilities

• Identify care coordination needs with families to support best outcomes.
• Coordinate family-based services in home, school, and community.
• Maintain documentation and participate in program evaluation.
• Engage families and build partnerships with community resources.
• Ensure services meet safety and treatment needs and are the least restrictive.
