Job summary

Hartford HealthCare’s Connecticut office seeks a Connections Coordinator for the Stronger Families, Stronger Future program. You will screen families prenatally/postnatally, provide ongoing support, and coordinate referrals to home visiting services, prenatal groups, and community resources.

Responsibilities include data entry in EPIC/ECIS, collaborating with social workers, and recruiting families for bilingual prenatal groups. A stable role within Connecticut’s leading health network.

Qualifications

• High School Diploma required; Associate degree preferred in health services
• 2+ years in human services or working with infants/toddlers families
• Experience with hospital EPIC systems and community-based services preferred
• Bilingual English/Spanish encouraged

Responsibilities

• Screen prenatal/postnatal families using standardized tools to identify needs
• Provide follow-up calls for 3–6 months postpartum with emotional support and resource access
• Enter and maintain screening data in ECIS, APRICOT 360 and EPIC; prepare weekly/quarterly reports
• Collaborate with hospital social workers to connect families with community services
• Build connections to parenting resources within the site and refer to statewide programs
• Recruit families for prenatal groups; manage attendance and evaluations
