Qualifications

• Bachelor’s degree in human services, social services, psychology, sociology, behavioral health, or a related field preferred; or an equivalent combination of education and relevant human-services experience.
• Minimum of four years of relevant human-services experience if no bachelor’s degree.
• Must be at least 21 years of age.
• Experience with SED Waiver, Medicaid, and community resources is preferred.

Responsibilities

• Wraparound facilitation and case management for children and families in the SED Waiver program.
• Coordinate family-driven services addressing behavioral health, education, social, and community needs.
• Facilitate treatment team meetings and ensure plans of care are implemented and documented.
