Job summary

National Youth Advocate Program in Jacksonville, FL seeks a dedicated Case Manager to advocate for children and families in our community. You will work closely with youth, family members, foster parents, and partners to provide professional services and support toward safe, stable, and permanent outcomes.

The role requires flexibility for non-traditional hours and daily travel, with supervision from the Director of Case Management.

Qualifications

• Bachelor’s Degree in Social Work or comparable Human Services field from an accredited institution.
• 2 years of work experience working in direct service with youth and families strongly preferred.

Responsibilities

• Actively supports, represents, and extends the mission, vision, and values of the organization.
• Completes or revises the Family Risk Assessment with all family members that live in the home to assess strengths, needs, and risk of harm to the child.
• Maintains contact with families through regular and planned visitation.
• Provides case management, transportation, parent education, counseling, and community resource linkage.
• Prepares for and participates in administrative and court reviews of the case plan.
