PATH, Inc. seeks a Case Management Supervisor - Family Team in Philadelphia to lead the CYS Blended Case Management: FACT program, ensuring high-quality care and adherence to regulations.

The role requires strong supervisory skills, clinical leadership, and collaboration with community partners. Qualifications include a Master’s in Behavioral Health, at least three years of direct care experience, PA licensure, and a valid driver’s license.

Qualifications

• Masters degree in Behavioral Health Discipline.
• At least three years direct care experience in a behavioral health/human service program.
• PA Licensure in a behavioral health discipline.
• Driver's License and Vehicle required.

Responsibilities

• Oversee CYS Blended Case Management: FACT program operations.
• Ensure compliance with relevant DPW codes and OMH bulletins.
• Supervise and coach staff to maintain program fidelity.
• Ensure children, youth, and families access required services.
• Maintain 24/7 crisis coverage with timely response.
• Manage program budgets and resources and coordinate with partners.
