Job summary

PATH (People Acting to Help), Inc. seeks a team-oriented supervisor for the Children and Youth Case Management FACT program. You will oversee case management activities for children and families, ensure staff fidelity to the FACT model, and coordinate services across clinical and community resources.

Candidates should have a Masters in Behavioral Health, at least three years of direct care experience, PA licensure, and a valid driver’s license with access to a vehicle to support field duties.

Qualifications

• Master's degree in Behavioral Health discipline.
• Minimum of three years direct care experience in a behavioral health/human services program.
• Experience supervising staff is desirable.
• PA Licensure in a behavioral health discipline is required.

Responsibilities

• Operate the CYS Blended Case Management: FACT program.
• Oversee admission, assignment of cases and discharges for children and youth and their families.
• Ensure staff are trained in the FACT model and maintain fidelity.
• Provide supervision and ongoing coaching to Facilitators/Case Managers.
• Ensure access to psychiatric, medical, social, financial and other services.

Qualifications

Masters degree in Behavioral Health Discipline. At least three years direct care experience in a behavioral health/human service program. Experience serving children, youth and families, strongly preferred. Experience as a supervisor is desirable. PA Licensure in a behavioral health discipline. Driver's License and Vehicle required.

Specific Duties

• Effectively operate the CYS Blended Case Management: FACT program.
• Maintain and enforce compliance with DPW’s PA Code 55 Chapter 5221: Mental Health Intensive Case Management; Chapter 5223: Mental Health Targeted Case Management General Provisions; MH Bulletin number OMH-93-09 dated 1993: Resource Coordination: Implementation.
• Oversee admission, assignment of cases and discharges of children and youth and their families receiving Family Anchored Care Management.
• Ensure that staff are trained in the FACT model and up to date on all required training.
• Provide regular supervision, as well, ongoing coaching to Facilitators/Case Managers on the model to ensure fidelity.
• Ensure that children, youth and families gain access to needed psychiatric, medical social, financial and other appropriate services.
• Ensure team meets productivity standards for CYS case management/FACT services.
• Ensure staff availability 24 hours/7 days a week to provide crisis coverage which includes responding to phone calls within 10 minutes and providing face-to-face intervention as indicated to resolve a crisis or respond to a family member’s request for face-to-face service.
• Participate in the provision of 24 hours/7 days a week supervisory coverage to on-call staff as assigned.
• Oversee and manage petty cash and other financial resources specific to the Children and Youth Case Management FACT program.
• Provide direct services as needed, especially during staff vacancies.
• Develop collaborative relationships with a variety of community-based services, faith-based resources, as well as medical, vocational and educational organizations.
• Appropriately schedule and ensure appropriate usage of program vehicles.
• Carefully review, follow-up, add appropriate clarification and process all incident reports generated by the Children and Youth Case Management FACT Program in a timely fashion.
• Carefully review, follow-up, add appropriate clarification and process all BH Review Committee Minutes generated by the Children and Youth Case Management FACT Program.
• Participate in the development of proposals as requested.
• Monitor service delivery to ensure that each child/you and his/her family receive quality, effective and appropriate prevention and case management services.
