Job summary

Little Flower Children and Family Services of New York is seeking a Care Coordinator to oversee access to services and coordinate a patient’s plan of care. You will work with families and a multidisciplinary team to ensure appropriate care, discharge planning, and follow-up across Long Island.

The role requires a BA/BS with experience or a Master’s degree, or RN with relevant experience, plus the ability to travel as needed. Strong communication and computer skills are essential.

Qualifications

• Bachelor’s degree with two years relevant experience.
• Master’s degree with one year relevant experience or RN with two years relevant experience.
• Ability to work with a diverse population.

Responsibilities

• Obtain enrollment consents from the individual or guardian.
• Complete initial and ongoing needs assessments (CANS) to determine level of care coordination.
• Maintain overall management of the Individualized Plan of Care and coordinate services.
• Coordinate enrollee’s provision of services and monitor adherence to treatment.
• Document effectively in Care Coordinator Portal (MAPP) and EHRs.
• Maintain contact with participant and families; conduct meetings as required.
• Act as advocate for clients within the agency and with external providers.
• Promote wellness by linking enrollees with resources and services.
• Participate in care planning meetings as part of an interdisciplinary team.
• Identify community resources and manage referrals, access, and follow-up.
• Assist with discharge planning and post-discharge services; arrange appointments and transportation.
