Family Support and Care Manager
Salary Range:
$37-$40k

Location(s): Lower East Side, NY Travel Required

Essential Duties and Responsibilities:

This role of the Family Support and Care Manaer is responsible for the overall coordination of services to meet the goals of the client/families focused plan of care. This is accomplished by building relationships with the caregiver(s) and care team and acting as a liaison to other agencies and ensuring they receive all necessary services and implementing the plan of care. Our aim is to provide the highest quality of care management services by complying with regulatory guidelines and meeting agency standards.

  • Obtain OPWDD(Office for People With Developmental Disabilities) services appropriate to the needs of the child and family.
  • Assist families with Medicaid application as needed.
  • Help families transition to a CCO(Care Coordinator Organization) for OPWDD.
  • Perform related duties as assigned by the Supervisor.
  • Maintain compliance with all company policies and procedures.
  • Other job-related duties as assigned.
Education and/or Work Experience Requirements:

Associate's degree in Psychology, Sociology, Social Work, Human Services or related field or a minimum of 2 years of job-related experience providing case management or medical focused care coordination services to individuals with special needs.

  • In-person in our Manhattan office, with travel once training is complete.
  • Must be bilingual Mandarin and/or Cantonese.
  • Each case requires bimonthly in-home visits, and the position can be hybrid once they have a full caseload.
  • Must attend all weekly in-person staff meetings.
  • Excellent verbal and written communication skills, including ability to effectively communicate with clients, and healthcare providers.
  • Need to use electronic health record systems, databases, and other computer-based tools to manage client information and maintain accurate records.
  • Strong organizational skills to help manage multi-family case load.
  • Willingness to meet with families in their homes or communities.
  • Computer proficiency (MS Office- Word, Excel and Outlook).
  • Ability to work independently and to carry out assignments to completion within the parameters of instructions given, prescribed routines, and standard accepted practices.
Physical Requirements:
  • Ability to travel to numerous locations to meet with clients, attend meetings and visit healthcare facilities.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
  • Must be able to lift and carry up to 35 lbs.
  • Must be able to talk, listen and speak clearly.
