Job summary

The Child Center of NY is seeking a Resource Coordinator (Care Manager) to support intake, enrollment, and ongoing assessments for families in NYC, with a focus on coordinating services across multiple providers.

You will conduct home or office visits, facilitate collaboration among clinicians, families, and community partners, and advocate for eligible services to maintain health and stability for enrollees. Home visits are required in Queens or Long Island areas.

Qualifications

• Bachelor’s degree in Human Services or a related field.
• 2 years of experience in Human Services, Psychology or related field with coursework in social work, child development, counseling or psychology.

Responsibilities

• Complete intake, enrollment, and monitor ongoing assessments.
• Coordinate interdisciplinary collaboration among providers, enrollees, families and supports.
• Advocate for individuals/families and refer to community resources.
• Conduct office/home/community visits to assess family needs and service delivery.
• Perform outreach to program candidates within the community.
• Coordinate services for families with children with medical/mental disabilities.
• Monitor compliance with Federal, State and local regulations.
• Use data management systems to monitor compliance.
