Qualifications

• Coordinate nurse home visiting: recruitment, scheduling, referrals, and post-visit connections.
• Manage databases and QA for patient data, productivity, and documentation.
• Track non-personnel expenses and maintain backup documentation.
• Create and update outreach materials; manage online presence and social media.
• Uphold Family Bridge's vision, mission, and values in daily work.
