Job summary

Bridgeport Hospital, part of Yale NewHaven Health, seeks a Program Coordinator to support Family Bridge services. You will manage office operations, coordinate nurse home visiting activities, and ensure data quality across multiple databases.

Ideal candidates have a Bachelor’s in social work or public health, 5 years of nonprofit program experience, and strong Microsoft Suite skills. Healthcare experience with maternal-infant services is highly valued. Fluency in Spanish is a plus.

Qualifications

• Bachelor's degree in social work, public health, or related field required; master's valued.
• Minimum of 5 years' programmatic or administrative experience in grant-funded or nonprofit programs.
• Healthcare experience and maternal/infant or nurse home visiting experience valued.
• Experience with Microsoft Suite and electronic records management.

Responsibilities

• Office management and handling of the program's main office phone and general inquiries.
• Oversee nurse home visiting administrative aspects: recruitment, scheduling, referrals, and post-visit connections.
• Manage multiple databases and systems tracking program service delivery, patient data, and documentation.
• Track non-personnel expenses and maintain backup documentation.
• Create and update outreach materials; manage the program's internet presence and social media.
• Embed Family Bridge's Vision, Mission, and Values in daily work.
