Job summary

Merakey is seeking a Family Based Case Manager for our Child and Family Services team in Warminster, PA, serving Bucks and Montgomery Counties. You will provide services to families, coordinate with schools, physicians, and other providers, and support children and adolescents in home and community settings.

This role requires a bachelor's degree in psychology or social work, at least one year of relevant experience, and a PA driver''s license.

Qualifications

• Bachelor's degree in psychology, social work or related discipline.
• At least 1 year of experience with children or adolescents.
• Valid Pennsylvania driver''s license and reliable transportation.
• Experience with crisis intervention is preferred.

Responsibilities

• Provide treatment services to families and education to enable home-based care.
• Maintain regular communication with families, schools, physicians and other service providers.
• Travel to client homes, other agencies and community resources to arrange and provide services.
• Support children, adolescents and families in community and home settings.
