The Denver Collaborative Partnership is seeking a Family Advocate & Facilitator to support families navigating multiple systems in Denver. You will provide peer support, coordinate ARCH staffing, and connect families to essential services, while maintaining client files and reporting as required.

This role emphasizes empowering families, effective communication, and collaboration with diverse partners to promote self-sufficiency and better outcomes for children and youth.

Qualifikationen

• Experience with at-risk youth and families in community settings.
• Experience facilitating group or staffing/family meetings.
• Preferred: parent or caregiver of a child/youth with mental health or co-occurring disorders.
• Knowledge of community-based resources for youth and families.
• Strong case management and administrative skills.
• Adaptable to change and able to innovate.
• Ability to listen, respect, and collaborate with diverse backgrounds.
• Flexible and able to respond to crisis.
• Time management skills.
• Willingness to be objective and balanced team member.

Aufgaben

• Provide peer support and navigation for families involved in multiple systems.
• Assist parents with meeting basic needs (healthcare, housing, food).
• Facilitate ARCH-related staffings and community partnerships.
• Schedule and confirm ARCH staffings with partners and families.
• Write staffing reports as needed.
• Attend court and meetings with families.
• Assist parents with connecting to community resources.
• Conduct home/community visits for progress follow-up.
• Assist with scheduling/attending appointments.
• Empower families to engage in treatment plans for self-sufficiency.
• Monitor and support youth and family progress.
• Maintain client files in the database.
• Mandated reporter of abuse and neglect to DHS.
• Provide case coordination for families.
