Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide comprehensive case management in community or office settings, coordinating services and supporting patients and families.

You will assist clients in accessing health services, financial aid, employment, education, social supports, and natural supports while developing tailored treatment plans and ensuring timely documentation. The role embraces a recovery-focused approach and a safe, supportive environment.

Qualifications

  • Highschool diploma or GED required; associates degree or higher in behavioral health preferred.
  • 6-month recovery from substance use and/or mental health preferred.
  • 1–3 years GMH/SU population experience required.
  • 1–3 years outreach and engagement in behavioral health required.
  • 1–3 years medical setting experience preferred.
  • Valid identification required.
  • AZ Driver's License preferred.
  • 39-month MVR preferred.
  • Arizona Fingerprint Clearance card required by policy.

Responsibilities

Job Summary

The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.

Skills/Requirements
  • Highschool diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
  • Minimum of six month of recovery from substance use and/or mental health disorders preferred.
  • 1-3 years of experience working with the GMH/SU population required.
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
  • 1-3 years of experience in a medical setting preferred
  • Valid identification required
  • Current AZ Driver's License (valid and in good standing), preferred.
  • 39-month MVR preferred
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Arizona Fingerprint Clearance card
