Segue Health is seeking a Nurse Practitioner to provide in-home post-discharge care for patients in Columbus, MS and surrounding areas. The role involves home visits, reviewing discharge documents, and coordinating with the care team to ensure seamless transitions from hospital to home.

The NP will assess and treat patients, prescribe medications as appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions, with travel within a 35-mile radius from home.

Qualifications

• Mississippi license and Medicare provider number required.
• Nurse Practitioner certification in good standing.
• Ability to travel within 35 miles of Columbus, MS.
• Strong communication and documentation skills; faith-driven approach.
