Job summary

Segue Health Management Corp is seeking an experienced Nurse Practitioner to deliver in-home post-discharge care across Dallas, University Park, Richardson, Garland, and surrounding areas. You will assess clinical status, interpret discharge information, and coordinate with home health professionals to ensure smooth transitions from hospital to home.

The role requires Texas licensure, NP certification, and the ability to travel within a 35-mile radius.

Qualifications

• Texas medical license active and in good standing.
• Nurse Practitioner certification in good standing.
• Ability to travel within a 35-mile radius from home to see patients in Greater Dallas area.
• Strong communication, clinical judgment, and documentation skills.
• Compassionate, faith-driven approach to patient care.

Responsibilities

• Conduct in-home visits to assess clinical status and provide post-discharge medical care.
• Review and interpret discharge summaries and progress notes.
• Collaborate with home health professionals and care team for coordinated care.
• Provide medical consultations, prescribe medications when appropriate, and oversee care plans.
