Job summary

Segue Health delivers faith-based, patient-centered home care in Denton, TX. We seek an experienced Nurse Practitioner to manage in-home post-discharge care, assess clinical status, and coordinate with the care team to promote recovery and reduce readmissions.

The NP will review discharge summaries, prescribe when appropriate, and oversee ongoing care plans for patients transitioning from hospital to home.

Qualifications

• Active Texas medical license required.
• Nurse Practitioner certification in good standing.
• Ability to travel within 35 miles of Denton, TX.

Responsibilities

• Conduct in-home visits to assess clinical status and provide post-discharge care.
• Review discharge summaries and progress notes for care planning.
• Collaborate with home health professionals to coordinate care.
• Provide medical consultations and oversee ongoing care plans.
