Job summary

Segue Health is seeking an experienced Nurse Practitioner to provide in-home post-discharge care management in Memphis and surrounding counties. You will assess patients, review discharge notes, prescribe medications as appropriate, and coordinate with home health teams to prevent readmissions.

The role requires Tennessee licensure, NP certification, Medicare provider number, and the ability to travel within a 35-mile radius from home.

Qualifications

• Tennessee medical license required.
• Active Medicare provider number required.
• Nurse Practitioner certification in good standing.
• Ability to travel within 35 miles from home to see patients.
• Strong communication, clinical judgment, and documentation skills.

Responsibilities

• Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
• Review and interpret discharge summaries and progress notes.
• Collaborate with home health professionals and care team for seamless coordination of care.
• Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery.
