Job summary

Optum Home & Community Care is hiring a Nurse Practitioner for in-home care in Columbus, GA. The NP will travel daily to patient homes, perform comprehensive assessments, manage acute and sub-acute conditions, and collaborate with an interdisciplinary team to ensure high-quality care.

The role requires NP certification, unrestricted state license, DEA licensure, and ability to work across varied home environments.

Qualifications

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
Active and unrestricted licensure in the practicing state.
Active and unrestricted driver's license with reliable transportation.
Current, active DEA licensure/prescriptive authority or ability to obtain.
Ability to gain a collaborative practice agreement, if applicable in your state.
Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties.

Responsibilities

Conduct comprehensive in-home assessments, including PMH, medication reconciliation, vital signs and physical exam.
Manage members through acute medical situations or exacerbations of chronic conditions.
Manage members during short-term sub-acute rehabilitation episodes.
Ensure accurate documentation, including ICD-10 conditions.
Communicate and collaborate with the interdisciplinary care team.
Conduct advanced illness and end-of-life conversations; provide counseling and education to patients and caregivers.
