Job summary

PATH (People Acting to Help), Inc. seeks a skilled leader to oversee the CYS Blended Case Management FACT program in the Philadelphia area.

You will supervise facilitators and case managers, ensure fidelity to the FACT model, and coordinate services for children, youth and their families. The role requires a Master’s in Behavioral Health, PA licensure, and at least three years of direct care experience in behavioral health/human services, with supervision experience preferred.

Qualifications

• Masters degree in Behavioral Health discipline with PA licensure.
• At least three years direct care experience in a behavioral health/human service program.
• Experience serving children, youth and families; supervision desirable.

Responsibilities

• Operate the CYS Blended Case Management: FACT program.
• Oversee admission, assignment of cases and discharges for families served.
• Provide regular supervision and ongoing coaching to staff on the FACT model.
• Ensure access to psychiatric, medical, social, financial and other services.
• Maintain 24/7 crisis coverage and timely response to phone calls.
