Job summary
AuthoraCare Collective is seeking a Nurse Practitioner for full-time, facility-based palliative care across Forsyth, Davie and Davidson Counties. The NP will provide consultative symptom management and advance care discussions for adults with serious illnesses in skilled nursing facilities and memory care settings.
The role requires CPR/BLS certification, collaboration with interdisciplinary teams, on-call coverage as assigned, and a comprehensive benefits package including PTO, holidays, health
Qualifications
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) required.
- NP provides consultative palliative care to adults in skilled nursing facilities, assisted living communities, memory care settings, and other contracted residential care environments.
- On-call coverage may be required as assigned.
Responsibilities
- Provide symptom management and goals-of-care discussions.
- Develop individualized palliative plans of care in collaboration with the care team.
- Coordinate across facility staff, physicians, pharmacists, hospice and home health teams.
- Assist in assessing for hospice eligibility or other care transitions.
