Job summary

Fleming Support Services seeks experienced and certified supported living coaches to help individuals locate and maintain housing while building daily living skills. You’ll assist with budgeting, meal planning, and personal-finance management, and support healthy, independent living in a team-based setting.

The role emphasizes collaboration with families and case managers, adherence to Medicaid/APD standards, and proactive documentation to support person-centered plans.

Qualifications

• Bachelor's degree in Education, Rehabilitative Science, Business, or a related field.
• Associate’s degree or 2 years of college and 2 years of direct experience supporting individuals with developmental disabilities.
• One year of college and 3 years of direct experience with the target population.
• A minimum of 4 years of documented, direct professional experience working with individuals with developmental disabilities (can substitute for college on a year-for-year basis).

• Maintain accurate, timely documentation in compliance with agency and Medicaid/APD standards.
