Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson seeks an Experienced Physical Therapist (PT) for our facility. The role includes assessment, treatment planning, documentation, and family education, under physician direction.

The ideal candidate has a PT degree, valid licensure, and experience with long-term or geriatric care. Collaboration with all departments is essential; evenings and weekends may be required.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on capacity and tolerance.
• Document and record patients' condition and educate patients and families in PT methods.
• Collaborate with all disciplines to provide patient-centered care.
• Report to work on time and maintain professionalism.
• Perform other duties by Director of Rehabilitation.
