Job summary

Sayville Rehabilitation & Nursing Center is seeking an Experienced Physical Therapist (PT) to assess and treat residents, develop individualized rehabilitation plans, and educate families. The PT will supervise PTA staff and collaborate with the care team to deliver patient-centered services in a long-term care setting.

Applicants must be licensed in the state, have a PT degree, and possess strong documentation and professionalism.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care/geriatric care preferred.
• Strong documentation writing skills.
• Experience with case mix index and PPS scheduling preferred.

Responsibilities

• Assess referred patients and provide direct rehab services.
• Establish and administer a treatment program with goals per patient capacity and tolerance.
• Document patient condition and progress; educate patients and families.
• Supervise and oversee Physical Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
