Job summary

Sayville Rehabilitation & Nursing Center is seeking an experienced Physical Therapist (PT) to assess and treat patients requiring rehabilitation within our facility. The PT will design and supervise a treatment program, document progress, and educate patients and families on therapy approaches.

Applicants should have graduated from an accredited PT program, hold a valid state license, and be capable of supervising PTAs. Experience in long‑term care or geriatric settings is preferred.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed and registered to practice PT in the state.
• Experience supervising Physical Therapy Assistants.

Responsibilities

• Assess referred patients requiring rehab and provide direct patient care.
• Establish and administer a structured treatment program with goals.
• Document patient condition and progress, including weekly notes and discharge summaries.
• Educate patients and families on therapy methods and expected outcomes.
• Collaborate with other disciplines to deliver patient‑centered care.
