Sayville Rehabilitation & Nursing Center is seeking an experienced Physical Therapist to assess and treat patients in a long-term care setting. The PT will develop goal-directed treatment plans under physician direction, document progress, and educate patients and families.

The ideal candidate will have an accredited PT degree, active state license, and experience with geriatric care. Strong documentation skills and teamwork are essential to provide high-quality patient care.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Must hold a valid state license and registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation and record-keeping skills.
• Ability to evaluate, document progress and discharge planning.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates exceptional patient service and professionalism.

Responsabilités

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment programs with goals set per patient capacity and physician direction.
• Document and record patient status and progress; educate patients and families.
• Coordinate with other disciplines to deliver comprehensive care.
• Ensure timely punctuality, professionalism and adherence to regulations.
