Job summary

Harvard University Health Services (HUHS) within CADM seeks a licensed clinical social worker to coordinate patient care across medical, social, emotional, insurance, and vocational aspects. Qualifications

Responsibilities

• Coordinate patient care with physicians, nurses, and social work professionals.
• Communicate updates to treating clinicians.
• Provide patient and family support for medical, social, emotional, and vocational needs.
• Document assessments and care plans in the EMR.
• Apply case management to promote cost-effective, high-quality care.
• Consult with manager on clinical, legal, ethical, and risk issues.
• Participate in quality assurance and performance-improvement activities.
