Job summary

SM Staffing seeks a Chief of Care Management to provide executive leadership and strategic direction for the care management workforce across the organization. This role champions accountability, person-centered care, and operational excellence with data-driven evaluation and continuous improvement, while aligning with the VP's to ensure seamless frontline operations and regulatory compliance.

Qualifications

• Master’s degree in health administration or related field is required or preferred.
• Minimum of 10 years related Human Service experience, with at least 5 in IDD.
• Three years of previous management-level experience.

Responsibilities

• Lead development and implementation of strategies to strengthen care management workforce and promote person-centered care.
• Provide high-level oversight to VPs of Care Management and workforce, ensuring clear expectations and growth.
• Foster accountability, transparency, and high performance across care management teams.
• Monitor data and analytics to evaluate outcomes and drive strategic, operational decisions.
• Collaborate with leadership to align care management with company-wide objectives.
• Identify opportunities for standardized processes and drive improvement initiatives from start to finish.
• Coordinate with other departments to drive system-wide improvements.
• Serve as strategic liaison with OPWDD and other agencies to address escalated issues.
• Attend Executive Oversight Committee and Strategic Initiative meetings.
• Ensure timely responses to inquiries within 24 hours and maintain efficient operations.
• Lead tasks and special projects and assist with coverage as needed.
• Ensure compliance with OPWDD regulations and Tri-County Care policies.
