Qualifications

• Bachelor’s degree in nursing required OR Master’s degree in social work required.
• Master’s degree in healthcare or related field preferred.
• Active South Carolina RN or Social Work licensure required.
• Certification in Case Management (CCM or ACM) preferred.

Responsibilities

• Directs, plans, and oversees daily operations of local case management services across the continuum of care in alignment with system case management policies, procedures, and models of care.
• Utilize the Case Management and Post Acute Care dashboards to monitor and analyze key performance indicators, identify trends and assist with patient placement and discharges.
• Collaborate with system and division leaders on performance improvement in initiatives aimed at reducing barriers and enhancing coordination of care.
• Ensure effective discharge planning processes are in place 7 days/week.
• Oversees high-cost resource utilization.
• Leads change management and standardization efforts across departments.
• Promotes a culture of accountability, collaboration, and professional development.
