Qualifications

• Bachelor’s degree in nursing or master’s in social work required.
• Master’s degree in healthcare or related field preferred.
• Active South Carolina RN or Social Work licensure required.
• CCM or ACM certification preferred.

Responsibilities

• Directs, plans, and oversees daily operations of local case management services across the continuum of care.
• Utilize dashboards to monitor KPIs, identify trends, and assist with patient placement and discharges.
• Collaborate with system and division leaders on performance improvement initiatives.
• Ensure effective discharge planning processes seven days a week.
• Partner with utilization management and clinical teams to optimize patient flow.
• Support budget development, financial monitoring, and resource allocation.
• Identify strategies to optimize cost efficiency while maintaining quality.
• Oversee high-cost resource utilization and lead operational improvement initiatives.
• Collaborate with Case Management, Social Work, and clinical teams to manage complex patients.
• Facilitate interdisciplinary case reviews and barrier-resolution meetings.
