Qualifications

• Active State Licensure as a Physical Therapist.
• Experience developing individualized treatment plans.
• Ability to supervise PT assistants and support staff.
• Excellent communication with patients, families, and health teams.

Responsibilities

• Consult with patients to assess physical condition and medical history.
• Diagnose movement dysfunction and observe patient motion.
• Develop and execute individualized treatment plans.
• Provide therapeutic massage and stimulation to ease pain.
• Maintain patient records and track treatment goals.
• Document regulatory and clinical requirements in EHR.
• Coordinate with supervisor and health team on patient progress.
• Supervise PT assistants and rehabilitation staff.
• Complete discharge summaries per policy.
