Job summary

The Center for Urban Community Services (CUCS) is a leading non-profit provider of housing and social services in New York City. The Per Diem Case Manager will work in the evenings, overnights, or weekends to support clients and ensure the well-being of the recipient census at a program site.

Responsibilities include helping clients obtain housing, supporting program operations to meet contractual goals, and providing direct services including medication management.

Qualifications

• 1 year related experience
• Appropriate verbal and written communication skills
• Some college preferred; A. may substitute for experience

Responsibilities

• Assist with program operations and meet housing placement goals as contracted.
• Provide direct clinical services and assist with medication management; support overall program goals.
