Job summary

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field preferred.

Responsibilities

• Conducts pre-admission assessments.
• Interviews client, family, and collateral sources to verify intake criteria.
• Reviews admissions against criteria and policies.
• Engages referral sources for client support and coordination with healthcare professionals.
• Documents activities, contacts, and appointments in the CRM.
• Verifies insurance benefits for admissions and coordinates with insurers.
• Maintains waiting list and facilitates admissions scheduling.
