Job summary

Responsibilities

• Coordinate discharge planning for patients/residents.
• Participate in interdisciplinary team meetings and quality improvement activities.
• Provide counseling and support to patients, residents, and families.
• Ensure satisfaction and compassionate interactions with residents and visitors.
