Parker Jewish Institute for Health Care and Rehabilitation is seeking a full-time Hospice Social Worker (LMSW) to join the hospice department in New Hyde Park, NY. The role reports to the Hospice Administrator and focuses on delivering comprehensive psychosocial and bereavement support to patients, families, and caregivers.

The position includes field visits in Queens and Nassau counties, with in-patient visits at Parker's facility.

Competenze

• Master's degree in Social Work required.
• Active NYS Social Work License required.
• Prior experience as a Social Worker in a community health, hospice or inpatient setting.
• Valid New York State driver's license and ability to make field visits in Queens and Nassau counties.

Hospice Social Worker, LMSW

Full Time

The Parker Jewish Institute for Health Care and Rehabilitation is seeking a full time Social Worker to join the team in the Hospice department.

Position Overview

Reporting to the Hospice Administrator, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for performing initial psychosocial, emotional, spiritual and bereavement assessments, ongoing assessments, and assists in the development and implementation of goal-directed IDG care plan while serving as an advocate for the patient and family members.

This position will include field visits in Queens and Nassau counties, in addition to in-patient visits at our facility.
