Job summary

Included Health seeks board-certified Nurse Practitioners licensed in Kentucky with multi-state licensure. You will deliver virtual comprehensive care to patients of all ages, in a high-volume Urgent/Primary care setting, collaborating with provider and support teams.

Minimum 3 years post-training in urgent or primary care is preferred; active RN compact license and national FNP certification required to support a broad range of conditions across all ages.

Qualifications

• Graduate of an accredited Family Nurse Practitioner program (Master’s or Doctorate).
• Active NP license and RN license in compliance with state requirements.
• National certification as FNP (AANP/ANCC).
• Active RN compact license and multi-state licensure (5+ preferred).
• Minimum 3 years in high-volume Urgent Care or Primary Care.
• Experience caring for newborns to adults; integrated practice exposure.

Responsibilities

• Conduct age-appropriate history and virtual exams.
• Diagnose, treat, and manage a broad range of conditions across lifespan.
• Order/interpret diagnostics and document treatment plans promptly.
• Manage acute, chronic, and mental health in pediatric and adult patients.
• Coordinate referrals and educate patients and families on plans.
• Maintain current knowledge of best practices and evidence-based care.
• Provide cross-coverage for lab results and scheduling flexibility.
