Job summary

Ecumen is hiring a Triage Nurse, RN to support our Assisted Living and Hospice teams in Shoreview, MN. This PRN/on-call position focuses on telephone triage to assess and guide patient care, with primary evening availability.

Responsibilities include documenting assessments and following clinical protocols to improve outcomes. Applicants must hold MN RN licensure and have caregiver experience; this role may involve on-call shifts and weekend coverage.

Qualifications

• Must be licensed as a Registered Nurse in MN.
• 1 year of care management experience in home care or hospice.
• Strong clinical and relational skills.
• Ability to analyze situations and implement solutions.
• Ability to handle multiple priorities and high stress situations.
• Proficient with Microsoft Office Suite

Responsibilities

• Provide telephone triage nursing guidance to patients and families.
• Accurately document symptoms, assessment, advice, and responses in the EHR.
• Follow policies, procedures, and protocols to ensure consistency and quality of care.
• Follow up on situations representing higher risk or need from family or staff.
• Maintain high standards of professional nursing ethics and practice.
