Job summary

Ecumen is hiring a Triage Nurse, RN to support our Assisted Living and Hospice teams. The role provides telephone triage nursing, documents symptoms and advice, and follows policies to ensure high-quality care.

On-call/PRN with evening and weekend shifts; the targeted pay range is $36.00–$41.00 per hour. Includes benefits based on hours worked, and a commitment to compassionate elder care.

Qualifications

• Must be licensed as a Registered Nurse in MN and must maintain licensure or obtain within 60 days of hire.
• 1 year of care management experience in home care or hospice
• Strong clinical and relational skills
• Ability to analyze situation and implement solutions

Responsibilities

• Independently provides professional nursing services over the telephone for Ecumen patient and resident care; documents symptoms, assessment, advice, and response.
• Follows up on situations representing higher risk or need from family or staff.
• Analyzes data to improve services and outcomes; designs workflows and participates in quality improvement activities.
• Provides high level nursing support for patient care questions at assigned site, including incident follow-up.
• Documents in multiple electronic health records with accuracy.
