Job summary

Polaris Pharmacy Services in Ohio is seeking a Staff Pharmacist to handle day-to-day dispensing for long-term and post-acute care. You will adhere to quality standards and regulatory guidelines while supporting patient care across shifts.

The role requires a PharmD/BS in Pharmacy, active state license, and 2+ years acute-care experience. Strong teamwork, communication, and flexible scheduling are essential.

Qualifications

  • Active pharmacist license in the state of practice.
  • Management experience preferred.
  • Pharmacy degree from an accredited school.
  • 2+ years acute-care or 1 year as production pharmacist preferred.
  • Experience with IV admixtures and therapy desired.

Responsibilities

  • Verify completeness/accuracy of new/refill labels and orders.
  • Maintain resident med histories and current meds data.
  • Assist in ordering/receiving/Storage of controlled substances.
  • Reconcile inventory for scheduled II drugs per regs.
  • Provide clinical answers to drug-related questions.
  • Document communications in medical records.
  • Prepare and supervise sterile IV drug products.
  • Cover evenings, weekends, and holidays as needed.
  • Follow HIPAA and other regulations.
  • Other duties as assigned; adapt to business needs.

Job Summary:

The Staff Pharmacist is responsible for the day-to-day operational duties associated with the dispensing of prescription medication as required to meet the needs of a long-term/post-acute care pharmacy. The Staff Pharmacist must adhere to the company's quality and customer service standards, policies and procedures, as well as the State and Federal guidelines.

SCHEDULE: 2pm - 10:30pm, working every 3rd weekend
Duties/Responsibilities:
  • Verification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits and IV medications
  • Maintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)
  • Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugs
  • Work with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulations
  • Provide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customers
  • Document all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
  • Preparation and/or supervision of all sterile IV drug products or solutions
  • Provide evening, weekend, and holiday coverage during rotation and as required by management
  • Follow all applicable government regulations including HIPAA
  • Other duties as assigned; Job duties may vary depending on business needs
  • Responsible for the flow of work and appropriate outcomes
QUALIFICATIONS/COMPETENCIES:

Required Skills/Abilities:
  • Perform accurately and efficiently when dispensing prescription medications
  • Ability to work flexible hours, which may include evening, weekends, and holidays
  • Experience using FrameworkLTC
  • Experience using Microsoft Word, Excel and Microsoft Outlook
  • Excellent interpersonal skills and well-developed verbal and written communication skills with English as the primary language.
  • Ability to work independently as well as working in a team environment, meets deadlines, establish priorities, and be flexible, work under pressure with multiple tasks. Must have strong organizational, customer service, and time management skills, ability to problem solve.
  • This position must exhibit maturity in judgment
EDUCATION and/or EXPERIENCE:
  • Current Pharmacist license active and in good standing in the state of practice
  • Management experience preferred
  • Bachelor's degree or Pharmacy Doctorate from an accredited School of Pharmacy
  • Minimum of two (2) year's recent experience in an acute-care setting or one (1) year experience as a production pharmacist preferred (not required).
  • Experience and knowledge base of intravenous admixtures services and intravenous therapy desired.
PHYSICAL DEMANDS

  • May sit or stand seven (7) to ten (10) hours per day
  • The employee is occasionally required to sit; climb or balance; and stoop, kneel, bend, walk
  • May be necessary to work extended hours as needed

HOLIDAY & PTO POLICY
  • Paid holidays are provided annually, with 6 days offered each year, along with 5 sick days.
  • Employees earn up to 10 PTO days each year, with rollover options and milestone bonuses.
  • Employees have the option to cash out up to 10 PTO hours each quarter for added financial flexibility.

Please note, because we are a pharmacy most of our locations are open 24-hours a day, 7 days a week and therefore schedules may change as determined by the needs of the business.
