Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. The RN III role involves clinical triage, care coordination, patient education, and leadership responsibilities within the care team.

The position is part-time with an evening shift, offering meaningful work beyond the bedside. You will collaborate with providers, care managers, pharmacists, and other health partners to improve outcomes and patient experience.

Qualifications

• BSN strongly preferred or graduate of an accredited RN program.
• Advanced certifications related to specialty (e.g., IBCLC) or graduate degree preferred.
• Active and unrestricted RN license in Michigan; CPR certification.

Responsibilities

• Triage patient needs by phone and in-office visits.
• Provide clinical guidance, education, and care coordination.
• Lead and support clinical staff with hiring and engagement.
• Manage referrals, test results, and prior authorizations.
• Support transitions of care and post-discharge follow-up.
• Collaborate with care teams and healthcare partners.
• Identify preventive care needs and assist with chronic disease management.
